ELIGIBILITY REQUIREMENTS
- ACT with a score of 18 or higher taken within the last 5 years or has taken the “Accuplacer Test” offered at the SGU Science Center arranged by Patrice Wright in the Foundational Studies Department (#605-856-8137). Contact to set up an appointment to take the “Accuplacer Test” if needed
- Completion of all Year 1 LPN prerequisite classes. As indicated on the status sheets (refer to status sheet tab), asterisked (*) classes must be completedwith a B average (80%) or above
- Cumulative grade point average of a 3.0 on a 4.0 grading scale
At the time of interview, GPA MUST reflect a 2.8 average (or above).
At time of acceptance, GPA MUST reflect a 3.0 (or above) - Maintain a current Certified Nurse Aid (CNA) or Medication Aide license.

- 1) Application Form: In order to apply to SGU LPN Program, you must submit the application form and the supporting documents (personal essay, immunization form, physical assessment form, and work or educational reference form) by (last working day in January, each year) by 4:00pm CST. Application form and supporting documents are located under the “Application Process” tab.
High School and College Transcripts: Official transcripts need to be submitted to SGU Registrar’s Office located in the Student Services building, by(last working day in January, each year) by 4:00pm CST. Transcripts must have official school seal stamp. Unofficial transcripts will not be accepted. - 2) TEAS V Test: After we have received your application form and supporting documents and it is determined you have met eligibility requirements, you will be notified by our Administrative Assistant to come to the nursing department to “check out” a TEAS V study manual as well as set up a directed TEAS study session appointment with a given tutor. This is mandatory. Once you have completed your study sessions with the tutor, you will take the computerized TEAS exam at the Nursing Department. You must pass the TEAS V exam with a 50% total score or better, or you will have to repeat at your own expense. An approved TEAS V score must be completed by last working day in February (each year) by 4:00 pm CST. Your contact person for TEAS V arrangements is Marissa Bordeaux

- Interview: After the application form, supporting documents, transcripts, and TEAS V Test have been submitted & approved, you will be notified to come in for a department interview
COMPLIANCE REQUIREMENTS AFTER ACCEPTANCE INTO PROGRAM
- You will do a criminal background check prior to the beginning of clinicals. Must be approved for continuation
- Full immunizations must be completed by 1stof August, each year by 4:00 pm CST (continuation in the program is conditional based on completion of the required immunizations). Note: several of the immunizations are multi-series and require lapses of time between doses, plan appropriately.
- Maintain a 3.0 (80%) in all LPN classes
